Detect early and treat as soon as possible

Shirley, 61, participated in a program through a district health center last month and was diagnosed with diabetes after receiving screening, consultation and laboratory tests at the private medical market with government funding.

"My family doctor explained to me that my glycosylated hemoglobin is high. Although I don't need to take medicine for the time being, I need to pay attention to my diet and control my condition."

She now maintains a balanced diet and eats less of her favorite ice cream and desserts.

"I used to eat a lot of meat, and I would eat it all the time if I liked it. Maybe I was young and didn't worry about getting sick. Now I usually eat more than two bowls of vegetables, and I try not to buy desserts."

Government funding Shared governance

Under the Chronic Disease Shared Treatment Program, participants are matched with a family doctor, and the government will subsidize part of the costs of screening consultations, drugs, tests, nurse clinics and allied health services. Participants only need to bear a certain co-payment. Currently, about 500 family doctors participate in the scheme, and more than 70% of them have a co-payment of $150 or less as recommended by the government.

Diabetics like Shirley or other people with high blood pressure or high blood sugar will receive up to four to six government-subsidized consultations from their family doctors every year. The district health center will also arrange allied medical services for them based on the diagnosis results of the family doctor.

Professional team comprehensive care

Wong Tai Sin District Health Center Nursing Co-ordinator Wang Diyun said that the inter-professional team consists of family doctors, nurses, nutritionists, physiotherapists, optometrists and podiatrists.

"They play different roles in the management of chronic diseases, including nutritional consultation, physical fitness and exercise guidance, fundus examination and podiatry treatment, etc."

Patients can also participate in classes to learn about pathology, the importance of medications, and common side effects.

Get screened and the risk is controllable

According to the Health Bureau, about 40% of patients with diabetes or hypertension are unaware that they have related chronic diseases.

Lin Yonghe, a family doctor participating in the program, pointed out that 45 years old is the ideal age for screening and hopes that eligible citizens will receive screening as soon as possible.

"Diabetes and high blood pressure are invisible killers. There are no symptoms in the early stages, but they may gradually lead to heart disease, stroke, kidney failure, eye lesions, sexual function or brain degeneration, etc."

He believes that the dedicated medical team of the chronic disease co-treatment plan can provide patients with more careful and effective care.

As of the 16th of this month, more than 17,300 people have participated in the chronic disease co-treatment program and been matched with a family doctor. More than 11,000 of them have been evaluated by a family doctor, and nearly half have completed screening. More than 30% of those who completed the screening were diagnosed with high blood sugar, diabetes or hypertension and required further follow-up or treatment.

The Health Bureau stated that starting from the first quarter of this year, eligible citizens can not only register to participate in the program at district health centers or district health stations, but can also register directly at clinics through invitations from family doctors.
