从疾病治疗到全生命周期健康管理医疗范式的转型

摘要： 随着人口老龄化与长寿时代的到来，医疗体系面临前所未有的挑战。本文分析了当前医疗模式在应对长寿社会健康需求方面的局限，包括服务碎片化、资源配置不均与医保可持续性危机。基于价值医疗与整合医疗理论，提出全生命周期健康管理范式，强调三大转型路径：预防先行、整合协同和多元支持。在这一范式中，全科医生作为健康“守门人”发挥关键作用，通过前移健康干预关口、提供全面健康筛查和加强社区健康服务，推动慢性病全程管理与医养结合服务，并借助多层次医疗保障、健康信息互联与协同治理机制实现系统支持。这一医疗范式转型是实现“健康中国 2030”目标的重要途径，需要理论创新、实践探索与政策协同共同推进。

Abstract: With population aging and the advent of longevity era，healthcare systems face unprecedented challenges.This paper analyzes the limitations of current medical models in addressing longevity society's health needs，including service fragmentation，unbalanced resource allocation，and healthcare sustainability crisis. Based on value-based healthcare and integrated care theories，it proposes a whole life cycle health management paradigm emphasizing three transformation pathways：preventive intervention，integrated coordination，and multi-dimensional support. In this paradigm，general practitioners play a key role as health “gatekeepers” by advancing health intervention points，providing comprehensive health screening，and strengthening community health services，promoting chronic disease management and medical-nursing integration，and achieving system support through multi-level medical security，health information interconnection，and collaborative governance.This medical paradigm transformation is an important path to achieving the Healthy China 2030 goals，requiring theoretical innovation，practical exploration，and policy coordination.
